  • /premium-agent-orchestration now survives spawn substrate failures: before fanning out to subagents, the skill probes the substrate with a trivial agent and confirms real output. If the probe fails, the premium lead falls back to serial execution rather than aborting or restarting shared infrastructure mid-run.
  • Multi-agent sessions that previously stalled or lost output on mid-run fork failures (ENXIO, phantom spawn) will now degrade gracefully and complete the task single-threaded instead of hanging.
  • Backoff is used on spawn failure instead of retry loops, preventing respawn spirals that could consume tokens or destabilize the session.
