- update PlantUML architecture documentation to describe idmap-based linking flow
- remove outdated standalone-linker-oriented wording
- align docs with current parser-driven sidecar output model and data flow

Replace all references to plantuml_links.json / standalone linker with the idmap sidecar flow. tooling_chain.puml updated to reflect the new pipeline.
